Forms

Referees will need the following forms to submit reports to NorCal Premier. All NorCal discipline is now run through GotSport, and all referees will need a GotSport account to submit reports. If you do not have a GotSport account you will be prompted to create one when you click on the links below.

 

Send Off Reports

Send off Report

A Send-Off Report must be filed within 24 hours of issuing a red card in any NorCal match. Failure to submit a send-off report will result in a fine for the hosting club. Repeated failure to submit send-off reports may result in further club sanctions from NorCal Premier.

  1. Please complete the form for any player or coach that you sent off in your match.
  2. Submit one form per individual that is sent off.

To submit a send-off report you must have a GotSport account. If you do not have a GotSport account you will be prompted to create one when you click the link (screenshot below). If you already have a GotSport account as a referee, make sure you use the email associated with your referee log-in.

Once you have logged in you will be prompted to select the event and match number that this report is tied to. Make sure you select the correct event name (located on the match card), and the correct game number (also located on the match card).

After you enter the match # and hit continue, the form will automatically pull in the team information from the match. You will then have to select your team. Please select the HOME TEAM on this page.

Once you have selected the home team, please fill out all information on the final page related to the send off.

Incident Report

Incident Report

Use the Referee Incident Report to report any one of the following issues that happen during a NorCal match.

  • Abandoned/ Terminated Match
  • Match unable to start/ match wasn’t started on time/ halves shortened
  • Injury (Head injury, EMS Called, other major injury)
  • Poor/ Unsafe Field Conditions
  • Coach/ Player Pass Issues
  • Game card/roster issues
  • Sideline/ Spector Behavior Issues
  • Coach/ Technical Area Behavior
  • Mass Confrontation/ Fight
  • Other (anything else that isn’t usual or wanted in a soccer match)

Information provided in this form will be reviewed by the NorCal PAD Committee for potential sanctions against the club, team, coach, or individual responsible.
